The short answer
Base yourself in San Sebastian and visit Bilbao as a day out. The two cities are about 75 minutes apart by direct bus, which makes Bilbao an easy day and a poor place to spend nights you could spend in San Sebastian, where the food, the beach and the evenings are.
Then make one decision: whether Bilbao gets a day of its own, built around the Guggenheim and the old town, or whether you fold it into an organized coast day that adds the islet of San Juan de Gaztelugatxe. The first suits people who came for the art and architecture. The second suits people who want both Basque icons without a car, and who are fine with a lot of stairs.
Where to base yourself
San Sebastian is small, walkable and built around one of the best town beaches in Europe, and its Old Town has the densest concentration of good bars you will find anywhere. Its evenings are the point of a Basque trip. Bilbao is a larger working city with its own old town and market, and a very good museum, but most of what a short trip wants from it fits in a day.
The one case for splitting nights is a longer trip, five nights or more, where one night in Bilbao lets you see its old town in the evening and start the museum early. For three or four nights, stay put.
Two ways to do Bilbao
Bilbao and Gaztelugatxe sit an awkward distance apart by car and are close to impossible to link by public transport. So the choice is structural: either you go to Bilbao alone, by bus, and give it the day, or you let a small-group tour drive the coast road and do both.
Option one: give Bilbao its own day

Option two: the coast day with Gaztelugatxe
The Bilbao, Guggenheim and Gaztelugatxe day trip is a nine-hour small-group day from San Sebastian: the Guggenheim in the morning, lunch, then San Juan de Gaztelugatxe, the hermitage on a rocky islet reached by a stone causeway that Game of Thrones made famous.
Two things to know before you book it. First, the steps: 241 of them down to the hermitage and, more to the point, 241 back up, in whatever weather the Bay of Biscay is producing that day. Reasonable fitness is required. Second, museum time on a day like this is generous but finite; architecture people will be satisfied, and completists will want to come back. It is the right trade for anyone who wants both icons, has no car, and would rather not spend the day on buses.

Which option is right for you
Choose Bilbao on its own if the Guggenheim is a main reason for the trip, if anyone in your group has bad knees, or if you like to set your own pace.
Choose the coast day if you want Gaztelugatxe and Bilbao without renting a car, and you are happy to trade museum hours for coastline.
Do both only on a long trip. The coast day covers the museum briefly, and a second visit to Bilbao is a lot of bus time for a short stay.
